Sliding Scale
A limited number of sliding scale spots are available for clients experiencing financial hardship. These rates are determined based on income and individual circumstances. Please feel free to inquire if cost is a barrier to care.
Ketamine assisted psychotherapy dosing sessions and ketamine assisted EMDR are self pay and are not covered by insurance. Integration therapy without dosing may be eligible for out of network reimbursement, depending on the plan.
Insurance:
Dr. Montalvo is no longer in network with insurance plans and currently practices as an out of network provider. A superbill can be provided for clients to submit to their insurance company for possible reimbursement, depending on out of network benefits. Total cost of treatment will be dictated by how many services are required, which we will estimate during intake meeting. Package pricing is also available for 4 or 6 KAP sessions.

Though each plan is individualized, we commonly recommend a commitment to at least 3 KAP sessions, as this medicine tends to have a cumulative effect, and multiple treatments prove the greatest benefit.

Insurance and KAP
At this time, Ketamine-Assisted Psychotherapy (KAP) sessions are not covered by insurance, as they are considered an out-of-network or elective service by most providers. Because of this, I am unable to bill insurance directly for KAP. However, I can provide a superbill that you can submit to your insurance for potential out-of-network reimbursement. Coverage will depend on your individual plan

Prescriptions for Ketamine
As a psychologist, I do not prescribe medication. For Ketamine-Assisted Psychotherapy (KAP), I refer clients to trusted medical providers who conduct their own evaluations and prescribe ketamine when appropriate.
