Sliding Scale
A limited number of sliding scale spots are available for clients experiencing financial hardship. These rates are determined based on income and individual circumstances. Please feel free to inquire if cost is a barrier to care.
Insurance:
Dr. Montalvo is an in-network provider with Aetna and Optum/United Healthcare for clients located in New York, Florida, and California If you are a member of either plan, your sessions may be covered in full or in part, depending on your specific benefits. Please contact your insurance provider to verify coverage and eligibility, or reach out directly with any questions.
Ketamine Assisted-Psychotherapy (KAP)
Total cost of treatment will be dictated by how many services are required, which we will estimate during intake meeting. Package pricing is also available for 4 or 6 KAP sessions.

We generally recommend 2-3 preparation sessions to collectively build the rapport and trust necessary for exploring deeper layers of your experience.

Insurance and KAP
At this time, Ketamine-Assisted Psychotherapy (KAP) sessions are not covered by insurance, as they are considered an out-of-network or elective service by most providers. Because of this, I am unable to bill insurance directly for KAP. However, I can provide a superbill that you can submit to your insurance for potential out-of-network reimbursement. Coverage will depend on your individual plan

Prescriptions for Ketamine
As a psychologist, I do not prescribe medication. For Ketamine-Assisted Psychotherapy (KAP), I refer clients to trusted medical providers who conduct their own evaluations and prescribe ketamine when appropriate.
